There are two ways to obtain a Birth and Death Certificate (birth or death occurred in Fulton County):
FEES
$25.00 - One search and one certified copy (if record is found)
$5.00 - For each additional certified copy (purchased at the same time)

Identification ("ID") is required for all requests, as prescribed by the Georgia Department of Human Resources

One piece of following ID (#1-18) is acceptable:
1. Driver's license, any state
2. Identification card (state issued)
3. Passport
4. Insurance card/policies
5. Military ID
6. Social Security Card
7. Bell South Bill
8. Credit Card
9. Payroll Check Stub
10. W-2 Form
11. Completed income tax return
12. WIC voucher
13. Monthly Medicaid card
14. Georiga Power Bill
15. Atlanta Gas Light Bill
16. Automobile title
17. Automobile registration
18. Voter registration card
Who can ask for a Birth Certificate?
- Registrant (person listed on birth record)
- Either Parent (name must be on certificate)
- Legal Guardian (must show proof)
- Grandparents (if parental, father's name must be on certificate)
- Adult Brother or Sisters (must be 18 years or older)
- Adult Child (must be 18 years or older)
- Spouse
- Legal Representative (of any of the above with proof)
Who can ask for a Death Certificate?
Relative Anyone with a legal interest (who knows facts about the death)
